Intake & Consultation
To get started, please contact my office at 561-424-6686. Leave a message, and office staff will contact you shortly. Once you speak with the intake coordinator, I will contact you personally to schedule your initial appointment (approximately 50 minutes).
Rates & Insurance
Please contact my office at 561-424-6686 for specific self-pay rates. I accept the following insurance carriers:
- Blue Cross Blue Shield
- Cigna
- United Health (Optum)
Payment
All clients will be required to leave a card on file before the initial session to cover any fees associated with services provided. Cards are charged only after a service is provided. Cash is also accepted for payment at the time of service.
Cancellation Policy
Please cancel or reschedule appointments at least 24 hours before the scheduled appointment time. Failure to do so will result in a fee for the full cost of the session, which will be collected on the day of the missed session from the card on file.
